Example NICU Admission Orders

Admit: to NICU, Dr. Sickman Attending
Diagnosis:  Prematurity, Sepsis
Condition: Critical

Vitals:  Routine NICU care and monitoring

• Vital signs q 1 hour
• Cardiorespiratory Monitor
• Pulse oximeter alarms: low 90, high 98
• Strict I/O

Lines

• UAV: run D10W at 3 cc/hour
• UVC: run D5W at 2 cc/hour
• total fluids calculated at 80 cc/kg/day

• Erythromycin ophthalmic ointment, O.C.
• Vitamin K, 0.5 mL i im
• Ampicillin 100 mg iv q 12 hours (100 mg/kg/day)
• Gentamicin 5.0 mg iv q 12 hours (5 mg/kg/day)
• Gentamicin trough one hour prior to second dose

• Labs sent from delivery room:  blood culture, chromosomes, type and cross, CBC with differential
• Labs to be sent at 12 hours of life:  electrolytes, calcium
• CSF (consent on chart)
• tube 1:  for culture and gram stain, VDRL, bactogens
• tube 2:  for glucose and protein
• tube 3: for cell count and differential
• Head sonogram at 7 days of life
