Preop Dx: Term IUP, failure to progress, [previous c/s, breech presentation]

Post-op Dx: Delivered viable female infant

Procedure: NSVD, 2o episiotomy and repair [LFT c/s]

Surgeons: Dr. Smith, MD, Dr. Dobbs, MD, Dr. Jones, MD, Mr. Bill, CCIII

Anesthesia: Epidural (GETT)

Findings:

viable female infant, 3500 grams in LOA position
Apgars 9/9, AF clear
Cord pH 7.31, BE -2, Hgb 15.2
Placenta delivered spontaneously, intact with 3 vessel cord. No nuchal cord
Uterus massaged to firm
Cervix/vagina inspected for tears and lacerations
2o episiotomy repaired with 2-0 and 3-0 vicryl
(vs. multiple uterine fibroids, etc.)
interoperative abx, meds given

Fluids: 1200 cc crystalloid

EBL: < 500 cc [1200 cc c/s]

Complications none

Specimens to Path: Placenta

Drains: none [Foley to gravity]

Pt. stable to RR, baby to NBN
